What happens when I leave care?

When you are about to leave care, lots of plans will be made about your future.  It is very important that you are asked what you want or else how will we get it right?

It is important that you are supported through this time.  You will be meeting new people and may have lots of changes, like where you live and starting further education or training.

The Somerset Leaving Care Service have produced a handy guide to give you lots of useful information about leaving care (see Library tab). 

A Leaving Care Worker will be available for every care leaver so you will be able to talk to them about your wishes and discuss what will happen to you.

 

Exit Interviews

If you are 12 or older, you can complete a questionnaire when you leave care.  There are 3 questionnaires. They aim to find out what worked for you and what did not while you were in care, and your experiences of going home or moving to live in the community.
